Anthony Pennell, a groundworker for R O Donnell Plant & Civil Ltd, suffered severe injuries on September 3, 2019, at a construction site in Fradley Park, Lichfield. Heavy drainage pipes, each weighing around 160 kg, fell on him while being moved by a telehandler. This incident resulted in multiple fractures to his pelvis, a fractured vertebrae, and a bleed on the spine.
The Health and Safety Executive (HSE) investigation revealed that both R O Donnell Plant & Civil Ltd and the principal contractor, Readie Construction Ltd, failed to ensure the operation was properly planned, supervised, and safely executed. Consequently, both companies were fined a combined total of £120,000.
