Stephen Picken was a demolition worker for Veolia ES (UK) Limited. Tragically, he died in an industrial accident on October 17, 2019, while decommissioning a North Sea gas rig at an onshore facility in Great Yarmouth. During the incident, a large piece of metal pipework fell and struck the mobile elevating work platform (MEWP) he was on, causing him to fall about 12 meters.
The Health and Safety Executive (HSE) identified serious failings in the planning and risk assessment for the work being carried out, which contributed to the accident. Veolia was fined £3 million for these failings.
